On Lok Lifeways, a PACE (Program of All Inclusive Care for the Elderly) organization has an opening in San Franciscofor a full time Family Medicine or Internal Medicine physician with experience in geriatric medicine. This is an opportunity to serve as a primary care provider practicing as part of an interdisciplinary team in a fully integrated environment. On Lok provides and coordinates the care services necessary for seniors to remain safely in the community. The physician candidate will provide direct medical care to PACE participants, including physical assessment, diagnosis and treatment, and participate in the development of an integrated plan of care to meet the individual's health and social needs. Duties will include following patients across the continuum of care, including inpatient settings, outpatient clinic, residential care facility, nursing facility, and patients homes. At least one year of experience in long-term, chronic care management, as well as experience in working in an interdisciplinary team is preferable. Active California medical license is required.
